Service Coordination
At Advocate Homecare, Service Coordination begins with a relationship. From your very first step into the Care coordination , your Service Coordinator gets to know you, your family, your goals, and what matters most to you.
The person who develops your Initial Service Plan stays with you for as long as you choose—providing one familiar, trusted point of support throughout your journey. No unnecessary handoffs. No starting over with someone new. Just consistent, person-centered support from someone who knows you.
Positive Behavioral Supports
For individuals whose behavioral responses may jeopardize their ability to live independently, our providers perform positive interventions. We help clients recognize ineffective behaviors and develop a detailed, holistic treatment plan to improve behavioral regulation.
Independent Living Skills Training
One-on-one support provided in the home or community to increase independence. We help individuals build practical skills such as cooking, shopping, banking, medication management, self-care, and use of community transportation.
Community Integration Counseling
Confidential counseling designed for individuals and families coping with altered abilities. We assist individuals with the emotional challenges of adjusting to community life after sustaining an injury and help navigate changes in personal relationships.
